Screening and Exams

    Physical Exams

      • Physical examinations are required by State Law in Grades K/1-6-11.
      • You may have your child examined by the school doctor or by your family physician.
      • It is often easier to make appointments when the students are home during summer vacation, therefore, for your convenience, forms are sent home near the end of school if your child is entering into grades K/1-6-11 for the upcoming school year.
      • If you choose to have your child examined by your family doctor, the completed private physical report should be returned to the school as soon as possible.
      • Forms can also be found under "School Health Forms" on my website.
    • Dental Exams
      • Dental exams are required by state law for Grades K/1-3-7.
